New Employee Enrollment
Welcome!
We are pleased that you have elected to accept a position with GRN
Community Service Board. You will receive an official appointment letter from the
Human Resources department with specific information about salary, enrollment, and
training requirements.
Your supervisor will notify you of the date and time to report for enrollment
processing. Any pre-employment requirements such as a medical examination or drug testing
also will be explained.
You will complete preliminary forms and select benefits prior to
enrollment day, so it is important to follow instructions provided below.
New employee enrollment and all initial training sessions take place at the Lawrenceville Center site, 175 Gwinnett Drive in the GRN Training Room, Room 251. If you need directions, refer to www.mapquest.com . The Center's address is 175 Gwinnett Drive, Lawrenceville, GA 30046. The center is located in the brown brick building behind the bank at the corner of Gwinnett Drive and Stone Mountain Street.
Each new employee is fingerprinted on enrollment day for submission of a criminal records background check. You are required to BRING WITH YOU a money order for $19.25 made out to Department of Human Resources to cover the processing cost.
Salaried employees report at 8 a.m. and hourly part-time employees report at 9 a.m. The GRN Training Room has an available refrigerator, microwave, and ice machine, if you prefer to bring your lunch. There are also a variety of fast food places within a reasonable driving distance.
You will be given time before lunch to go to our local public health
office for the fingerprinting process. In the afternooon on enrollment day, you will begin
the four full working days of training and orientation which must be completed prior
to your first day on the job. Consult your supervisor for reporting instructions for
your actual work site.
Pay periods are semi-monthly, from the first to the fifteenth and the sixteenth to the
last day of the month. Employees are paid on the 15th and the last day of the month. If a
payday falls on a weekend or observed holiday, the actual pay day is the last working day
prior to the weekend or holiday. New, salaried employees receive the first paycheck on the
next payday following hire. New, hourly employees receive the first pay check on the
second payday following hire for hours worked during the first pay period.
PRIOR to Enrollment Day
If your position makes you eligible for employee benefits:
ON Enrollment Day
Report promptly at the assigned time.
Bring the completed forms and Policy Acknowledgements with you to enrollment.
Bring your original Social Security card, your
current Georgia drivers license, and any other identification necessary to establish your
eligibility for employment. A list of acceptable documents is included on the Employment
Eligibility Verification (Form I-9). Your Social Security number will be verified through
the federal E-Verify system following your enrollment.
Bring a list with the full names, birthdates and Social Security numbers
of your spouse and dependents, if applicable. This information is necessary to complete
beneficiary election forms and to list any dependents to be covered by insurance.

Health Insurance
GRN Community Service Board offers health insurance through the State of Georgia's State Health Benefits Program (SHBP).
On the day of new employee enrollment, you will be required to make your decision on a health care benefits plan. Therefore, it is essential that you read and study the State Health Plan Decision Guide, which describes the details of the various plans. Full name
You will answer questions regarding your and/or your dependents use of tobacco products during the previous 12 months. If you enroll your spouse in the plan, you will answer questions regarding his/her eligibility for health care coverage through his/her employer. Please refer to the Decision Guide for details.

Information about the Flexible Benefits Program is available on the State Personnel Administration's web site www.spa.ga.gov. Click on "Flexible Benefits" for plan descriptions and rates.
You will
complete an options statement on enrollment day to select your benefits. If you elect life insurance or accidental
death/dismemberment insurance, the beneficiary election form requires the following
information for each beneficiary:
Each
employee has unique needs. The choices are
many. To make informed decisions, you
will need to study the information on each benefit. Once made, your benefit choices cannot
be changed until the next open enrollment period, which occurs each fall. So it is
important to choose carefully.
401k Retirement Benefits
GRN CSB contributes an amount equivalent to 7.5% of your salary into a 401k retirement account in lieu of payment into your Social Security account. Therefore, enrollment into the 401k plan is mandatory for eligible employees. Adding your own funds to your account via payroll deduction is optional. For more information about the 401k plan for employees of Community Service Boards, go to the Employees Retirement System web site www.ersga.org, In the drop-down list under the "FAQ's" tab, click on Peach State Reserves FAQ's and follow the links.
On the day of new employee enrollment, you will be given instructions for your 401k enrollment, selection of investment options, and designation of beneficiaries, all of which are done via an internet link. Should you fail to complete the process within 30 days of hire, you will automatically be enrolled into investments that are pre-selected based on your age.
Funds are managed by Peach State Reserves, the Deferred Compensation division of the Employees Retirement System. Accounts are serviced by CitiStreet, a contract administrator.
